If you are already enrolled in Original Medicare (Parts A&B) and have been diagnosed with an eligible chronic condition, you may qualify for a special kind of Medicare Advantage plan known as a Chronic Condition Special Needs Plan (C-SNP). Another type of Special Needs plan is for those on both Medicaid and Medicare, called Dual Eligible Special Needs Plan (D-SNP).

Medicare Supplement Insurance (Medigap) is extra insurance you can buy from a private health insurance company to help pay your share of out-of-pocket costs in Original Medicare (Parts A&B).

Medicare Prescription Drug Coverage (Part D) is optional insurance that helps pay for outpatient prescription medications.
